Well I sort of met khaosworks at GaFilk, but I don't think we ever did the formal "stick out your hand/paw/tentacle and shake" ritual.
And there are people on my LJ-friends list that have either chosen to not do the same in return, or have dropped me from their list for various reasons (most of these are due to friend-pruning). *sigh*
